Depending on your budget and expertise, you could look at putting a 
mail relay in front of your Exchange server to handle the filtering.  
If you're comfortable with Linux/BSD etc. you can build an extremely 
functional solution for pretty much the cost of your hardware.

If you're not familiar with anything but Microsoft servers, you could 
still acquire a relay/filtering appliance to do the same thing, 
though that would cost more.

Have a look at the CanIT appliance 
(http://www.canit.ca/appliance.php) for an example of what I mean.
