scanning remote hosts for connection information

Hello all,

I'm wondering if/how it is possible to obtain information about what open 
connections there
are on a remote host, i.e. what IP addresses is it connected to, and at 
what ports?  Is there a
way to do this with NMAP or similar scanners?  A related question; what 
experiences have any
of you had using something like NMAP to scan for activity on IRC ports 
(looking for bots, for instance)?
